The chart answers all three, because medicine has a shape in it: a small set of planets for the body and its care, three houses for disease and healing, and a divisional chart that says which part of medicine the native ends up in.

What medicine is in the chart
The sixth house is disease and service, and it is the first house of medicine: the doctor serves the sick, and a career in medicine is a sixth-house career before it is anything else. The eighth house is the body’s interior, surgery, crisis and research, the house of what is cut open and what is hidden. The twelfth house is the hospital, the institution behind walls, care given in confinement, and the loss the profession lives beside. The tenth is the profession, the fifth is diagnostic intelligence and judgment, and the ninth is the long higher learning that medicine demands more of than any other field.
The Sun is vitality, the body as a whole, and the physician’s authority; the Sun in the doctor’s chart is what makes patients do what they are told. Mars is blood, the knife, emergency and the nerve to act, and no surgeon’s chart is without it. The Moon is care, fluids, nursing, the public and the patient’s mind, and it is prominent in the charts of those who heal by attention rather than intervention. Jupiter is the physician’s wisdom, the ethics of the profession, and medicine as it is taught and counselled. Venus is aesthetics, fertility, the skin, and the comfort side of healing. Mercury is diagnosis, the laboratory, pharmacy and the communication of findings. Saturn is chronic disease, the old, the bones, and the long discipline of training. Rahu is the newest technology in the field and the foreign practice. Ketu is anaesthesia, the hidden surgery, research at the cellular level, and the alternative systems.
The doctor’s signatures
The Sun or Mars connected to the sixth house and the tenth is the primary signature: vitality or the knife joined to service and profession. A strong sixth lord connected to the tenth lord is the second, the service career that becomes a standing. The Moon and Mars in connection, by conjunction or mutual aspect, with either touching the sixth or eighth, is the classical combination for medicine, fluids and blood together, and it appears in the charts of physicians and surgeons alike. Jupiter aspecting the sixth or the tenth gives the ethical physician and the teacher of medicine. Mercury in the sixth gives the diagnostician. A strong eighth lord, or Mars in the eighth, gives surgery and research. Ketu in the sixth, eighth or twelfth gives the specialist in what others do not see: the anaesthetist, the pathologist, the radiologist. And the Sun or Mars in a kendra of the D10 confirms the lot.
Two of these is a chart that can hold medicine; three or more is a chart built for it. A chart with none is not a doctor’s chart, whatever the family says, and the honest reading at sixteen is worth more than the coaching fees.

The correspondences are tendencies, and a specialty is chosen by a dozen things the chart does not see. What the table does is settle the young doctor’s three options: the chart with Mars in the eighth and the one with the Moon in the fifth are pointing at different rooms in the same hospital.
The entrance examination, read as timing
The examination is an educational event and a competitive one, and it is read from the fourth, fifth and ninth houses for the learning and the sixth and eleventh for the competition and the result, with the education houses covered in the education guide. The attempts that succeed fall in sub-periods of the fifth, ninth, sixth or eleventh lord, or of a planet placed in those houses, with Jupiter transiting the fifth, ninth or the lagna as the trigger. An attempt made in a sub-period with no connection to those houses tends to fall short of the cut-off, and the pattern of one or two near misses followed by success when the sub-period changes is common.
Two things should be said plainly to families. The chart identifies the sub-periods in which the examination is most likely to go well and the ones in which it is not, and that is a real service, because it tells a family which year of coaching is the year to invest in. And no chart guarantees a seat. The competition is a sixth-house matter, the seats are finite, and a reading that promises admission has stepped outside what the method can support. The medical signatures above say whether the native belongs in medicine; the examination timing says when the door is most open; the native walks through it.
Nursing, allied health, pharmacy and alternative medicine
Many charts carry the healing houses without the degree. The Moon or Venus strong in the sixth or twelfth, with no Sun-Mars connection to the tenth and a ninth house too weak for the long training, is the chart of the nurse, the physiotherapist, the technician, the counsellor, and it is a healer’s chart in every sense that matters to the patient. Mercury in the sixth without Mars gives pharmacy and laboratory work. Jupiter or Ketu in the ninth with the sixth involved gives Ayurveda, homoeopathy and the traditional systems, and Jupiter in the sixth on its own often gives the physician of the old kind, consulted for wisdom as much as prescription. The reading that tells a family their child has a healing chart but not a surgeon’s chart, and names the allied field where the chart will thrive, prevents a great deal of misery, and it is the reading I give more often than the one that says medicine.
Government hospital, corporate hospital, or practice
The doctor’s chart also has a sector. The Sun and Saturn connected to the tenth give the government hospital and the medical college, the state’s medicine, with the combinations in Government Job Yoga in the Kundli. Timing a medical career
Entry into medicine, the seat and the degree, falls in fifth, ninth or sixth lord sub-periods. The years of training belong to Saturn’s and Jupiter’s sub-periods, and a Saturn sub-period during residency is the grind the profession is known for. Specialisation falls in a fifth or ninth lord sub-period, often Jupiter’s or Mercury’s. The first standing, the consultant post or the practice, falls in a tenth or seventh lord sub-period with Jupiter transiting the tenth. Surgery, for the chart that has it, expresses in Mars’s sub-periods and in the period of the eighth lord, and a surgeon’s best years are usually a Mars sub-period inside a supportive major period. The long career, the doctor still consulting at seventy, is Saturn’s, and a strong Saturn is what gives a medical career its length. D10 confirmation
The Dasamsa confirms a medical career when the Sun or Mars holds a kendra of the D10, when the D10 sixth or eighth is occupied by a medical planet, or when the D10 lagna lord is the Sun, Mars or the Moon. A rashi chart with the signatures and a D10 in which they vanish produces the doctor who administers, teaches or leaves, and the reverse produces the doctor who came to medicine by an unlikely route and stayed. A worked chart: a surgeon
A Cancer lagna chart, identifying detail removed. For this lagna Mars rules the tenth and fifth and is the yogakaraka, Jupiter rules the sixth and ninth, Saturn rules the seventh and eighth, Mercury rules the third and twelfth, the Sun rules the second, Venus rules the fourth and eleventh, the Moon rules the lagna. Mars sits in Aquarius in the eighth. The Sun is exalted in Aries in the tenth with Mercury. Jupiter is in Sagittarius, its own sign, in the sixth, with Ketu. The Moon is exalted in Taurus in the eleventh. Venus is exalted in Pisces in the ninth. Saturn is in Capricorn, its own sign, in the seventh. Rahu is in Gemini in the twelfth.
Count the signatures. The Sun exalted in the tenth: the physician’s authority, and a body that holds up under the profession’s hours. Mars, the yogakaraka, in the eighth: surgery, the knife in the house of the body’s interior, and the chart’s engine pointed at it. Jupiter in its own sign in the sixth with Ketu: the healer’s sixth, specialised, and the ethics of the profession intact. The Moon exalted in the eleventh: care that is paid for, and the patient’s trust. Venus exalted in the ninth: the long higher learning done well. Saturn in its own sign in the seventh: private practice, structured, late. Five signatures, a Mars-and-Sun chart, and the table says surgery.
Jupiter’s major period, the sixth and ninth lord’s, began in his mid-teens, and Jupiter’s own sub-period covered the entrance examination, cleared at the first attempt, which is what a sixth-and-ninth lord’s sub-period with the lord in its own sign does. Jupiter-Saturn was the undergraduate years, the grind, Saturn’s. Jupiter-Mercury, Mercury sitting with the exalted Sun in the tenth, brought the postgraduate entrance and the choice of surgery over medicine, which the Mars in the eighth had made before he did. Jupiter-Ketu, Ketu with the sixth lord in the sixth, was the residency, specialised and hidden, the years nobody outside the hospital saw. Jupiter-Venus, the eleventh lord exalted in the ninth, brought the senior post and the first real income.
Saturn’s major period began in his early thirties, the seventh and eighth lord’s, own sign, in the seventh, and it is a practice period: he joined a corporate hospital in Saturn’s own sub-period and built a surgical practice inside it. His surgical peak so far fell in Saturn-Mars, the yogakaraka’s sub-period, the planet in the eighth given its turn. The Sun’s exaltation in the tenth means he will run a department; Saturn in the seventh means it will be his own, eventually, and late. The chart had said surgeon at sixteen, when the family was hoping for medicine and would have accepted engineering.
What the reading cannot do
It cannot promise a seat, and it should not be used to keep a child in coaching whose chart shows none of the signatures. It cannot see the cut-off, the reservation category or the number of seats. It is not medical advice and has nothing to say about anyone’s health; that is a doctor’s work. It names a category and a lean within it, not a specialty, and a dozen things the chart does not see decide the rest. It depends on the birth time, since the sixth, eighth, tenth and twelfth are whole signs and a lagna error of one sign moves every signature. And where the chart shows healing without the degree, the reading should say so kindly and name the allied field, because a nurse’s chart in a surgeon’s coaching class is a common and avoidable unhappiness.
